The Society of Professional Designers

1.	Complete the application form;
2.	Deposit the application fee into the Society's bank account.
3.	Mail (registered mail please) the application form, or hand in the application by person, with the following attachments:
	A)	2 x Applicant's passport size photos (the photo must be taken within half a year from the date of application);
	B)	Photocopied evidence of your full name, date of birth, nationality and identification card number, (i.e. birth certificate, passport and / or identification card.
	C)	Photocopies of the applicant's gradation certificate and transcript, and evidence of work experience; and
	D)	Photocopy of the application fee bank deposit slip.
